[omniORB] giopServerThreadWrapper obsolete?

We got rid of it in preparation for a slightly different scheme for
doing the same thing, partly to reflect the change in the way omniORB
4 uses threads. Unfortunately, the new thing hasn't been put in yet.
Hopefully in the next couple of weeks.
